Crown Lengthening

Crown lengthening is a minor surgical procedure used to expose more of a tooth’s surface by removing or reshaping the surrounding gum tissue, and occasionally the bone. This treatment is often recommended when a tooth needs to be restored but lacks enough visible structure above the gumline. It is also used in some cases to improve the appearance of a smile.

What the Procedure Involves

Crown lengthening is typically done under local anesthesia. The periodontist carefully adjusts the gumline, and in some situations, may contour a small amount of bone to ensure a stable result. The goal is to expose enough of the tooth to support a crown, filling, or other dental work, or to create a more even gumline for cosmetic reasons.

The procedure usually takes about an hour and may involve one or several teeth. Recovery is relatively quick. Most patients can resume normal activities within a few days, with complete healing occurring over several weeks.

When Crown Lengthening Is Recommended

Tooth Restoration

If a tooth is decayed, broken, or worn down below the gumline, there may not be enough visible structure to support a crown or filling. Crown lengthening provides the necessary access by revealing more of the tooth.

Cosmetic Improvement

In cases of a "gummy" smile, when too much gum tissue covers the teeth, crown lengthening can expose more of the natural tooth surface. This leads to a more balanced and proportionate appearance.

Gum Pocket Reduction

Occasionally, crown lengthening is used to reduce the depth of gum pockets that are difficult to clean, especially in patients with periodontal disease. Removing excess gum tissue can improve access and reduce the risk of infection.

What to Expect During Recovery

Periodontal surgery provides several long-term benefits to patients suffering from advanced periodontal disease:

  • Some mild discomfort and swelling are common in the first few days.

  • Stitches are typically removed within a week, unless dissolvable ones are used.

  • You will be given detailed care instructions, including how to brush and eat during the healing process.

  • Complete healing may take one to two months, depending on the extent of the surgery and your overall health.

Follow-up visits help ensure proper healing and allow coordination with your general dentist if further treatment is planned.
